Hi, could someone who still remembers, could you answer the following questions I have about filling out entry/exit form. We're trying to fill it out ahead of time. We have Tonkin Travel arranged for VOA.
Q1: On the Entry and Exit Visa form, it asks for "Kind of Passport". What does that mean? What do I put there? I just have the regular US passport.
Q2: Also, it asks for "Name, address of office, organization or full name, relation, address of your relatives to contact or visit". What to put here? Name and address of first night hotel ok? Or maybe put Tonkin Travel?

For the passport question I'd put "Tourist". And I'd be inclined to, yes, put the agency name & address.
Some of these questions probably don't apply to everyone so your answers don't make much difference. I entered Vietnam on a train in the middle of the night and have no recollection of what I wrote which likely means it hardly matters. If they want to know something else you'll be right there to supply it, right?
