We're heading to Curacao and Bonaire in December for a couple of weeks. Although we usually don't rent cars when we travel, it appears to be the best option for these islands.
My question relates to insurance. Here in the US, we never purchase the add on insurance that is offered with a rental.
Is it necessary/advisable to purchase it on Curacao and Bonaire?

Depends if you had adequate cover in case of an accident from another source eg credit card company. We always purchase the CDW with car rental, no matter which location.
We rent cars in Curacao and in the round-abouts it gets pretty tight and the locals, especially on their way home from work 4-6 are aggressive and bump and run. It happened twice in our 3 visits. Most credit cards cover the first $1000 but once you are back home you cannot control how much the rental car spends on repairing their cars. Though it is $15/day I go with the Collisoin Damage waiver in Curacao. i had no need to rent a car in Bonaire.
