We're trying to decide among:
Anguilla: Viceroy or Malliouhana
Turks & Caicos: Regent Palms
St John: Caneel Bay
Nevis: Four Seasons
We're looking to travel in January. Our 3 year old is active & adventurous, but doesn't like large, chaotic environments, so we're looking for a solid, caring kids' program, heavier on personal attention than bells & whistles. (Plus we want to spend a lot of time with him as well) We like snorkeling, sunning, turquoise water and a bit of spa services.
I'm very appreciative for anyone's opinions.

Well, Four Seasons, Nevis has one of the BEST Children's Programs in the Caribbean - exactly what you are seeking - lot's of personal attention. The resort also has a fantastic Spa and there are plenty of places around the island you and your child can explore on your own when you want that time with him. Unfortunately the island won't have the snorkeling you crave nor does it have the bright turquoise waters that are probably in your mind's eye.
Malliouhana on Anguilla is closed so strike that from your list. I can't comment about Viceroy since I've never stayed there and I haven't read much about their children's program. The island has some of the best beaches in the Caribbean with dazzling white sand and bright turquoise waters. Snorkeling is somewhat limited and there's not a lot to "explore" around the island - it is rather flat, arid and featureless.
It's been a while since I've been to the T&C but topographically it is similar to Anguilla - flat & arid. I don't recall there being lots of places you can explore on your own that would interest a 3 year old but other contributors may be able to weigh in on that point.
That leaves, Caneel Bay on St. John. This rest may be perfect for what you are seeking. They have a decent Children's Program, there are 7 beaches on the resort offering some very good to excellent walk-in snorkeling, there's a lot to explore "off-resort" for that time alone with your child and they have Spa services available.
Of the places on your list, I think Caneel Bay fits your requirements to a "T".

I'm running into some trouble with Caneel Bay--it looks like they only have small rooms available, so we might go for one of the others. Since you have problems getting a room at Caneel Bay, I'd pick Turks and Caicos. It actually has everything you're looking for, and it's probably the best beach in the Caribbean for toddlers, and a great destination for families.
The best kids club there belongs to Grace Bay Club, followed by Seven Stars. RP has a kids club, has one of the best and prettiest spas in the Caribbean, and is popular with families, so there will be other toddlers to play. Throw in nice snorkeling, awesome restaurants and water excursions. Additionally, you can take your toddler to Sapadilla Bsy, which is a toddlers dream, they will be giddy. Made for toddlers, this beach is ankle to knee hi for hundreds of yards, and forms little sand bar islands at low tide, toddler will be soooo happy.
I love Anguilla, but no real snorkeling, and more adultish, I find TCI more toddler/family friendly.
I love St. John, if you'd like the equalavent of that you might want to check out Little Dix Bay on Virgin Gorda, a sister property.
I've never been to Nevis so can't comment.
I'd pick Turks & Caicos and stay at The Regent Palms . Great spa -- world class , on the cover of travels mags. Great on site restaurants ( breakfast included) , many dining spots nearby, kids club and snorkel not far away--can walk. The suites are beautiful and VERY clean !
Loved Nevis, but it takes forever to get to ( one plane to San Juan, another to St. Kitts, then boat to Nevis) so with a 3-year-old along, I can't recommend it.
I agree w/Blamona that T&C would be ideal. As noted, it's a popular destination for families, and has lovely accommodations, plus a gorgeous, long beach with calm, clear water.

We ended up booking for Nevis-four seasons- managed to get a 2-hop trip, nonstop to San Juan, then cape air to Nevis. We're very excited, and will take your advice & try T & C next trip.

You will love Four Seasons, Nevis...we had a ball there....even met Kelly Ripa & husband Mark . Had fun with them and dinner twice...
The Four Seasons in Nevis is beautiful, and the beach will be nice for your 3-year-old. Glad you could cut the trip time down a bit! Oh, and if you're golfers, you must try that course! One of the prettiest I've ever seen. Have fun
Your toddler will enjoy seeing the monkeys on Nevis. Ask at the 4S where and when the best time to spot them on the golf course.
I can also recommend Peak Heaven for an outing. They have a playground for children and it's just a lovely spot to walk around, see the amazing views, and have lunch. www.peakheavennevis.com.
The Hermitage is pretty family friendly, especially if you eat on the early side. That's if you want a change from the resort. They have pizza night on Fridays and Saturdays, as well as the regular menu. There are at least 4 beach bar/restaurants near the 4S now--good for a short walk on the beach and a nice lunch. Enjoy!
I'm definitely biased and live and word in world famous Grace Bay in Providenciales, Turks & Caicos - but must agree hands down with what Caribzack and Blamona are both saying. "...snorkeling, sunning, turquoise water and a bit of spa services..." - we've got it all (and more).

Sorry, I'd choose Nevis or Anguilla even though they are difficult to get to. Just one Caribbean lovers opinion.
