I can help with a couple of your questions, hopefully others can fill in the blanks.
It depends on how much time you have in Italy and when you're going. If you're only in Sorrento for just a few days, then your wish list is OK, again may be too much if you only have that time. To do all of it you need a week and you could include Paestum (more ruins).
The drive from Naples is usually on the autostrada and takes about 2 andf a half hours. It goes too quickly to see much. To take the "old" road, then you need a full day and it is worth it.
The Necropolis is well worth a visit.
All of this depends on the length and timing of your trip.

There is no all-in-one ticket for all of Italy. Some cities have passes for set periods of time (12 hours, 24 hours, 72 hours, and so forth).
It would help to know when you are going and how long you have in each location. Generally speaking, the things you want to do in AC can be done in 5-6 days. We enjoyed Capri very much but we stayed 3 nights and avoided the cruise ships and daytrippers which make it very crowded at mid day.
I think it is a good idea to take the train from Naples to Rome. You will not need (or want) a car in Rome, Florence or Venice.
It is good to have reservations for any special tours you want to take such as the Scavi tour in Rome. Also for the Borghese Gardens in Rome, the Uffizi and the Accademia in Florence.
Do a search on this board to get input on the Rome pass - we have never used it so can't comment.
Yes bookings essential for the Borghese and Uffizi. Book Vatican tickets here - http://biglietteriamusei.vatican.va/musei/tickets/do?action=booking. I would book for the Accademia in Venice to avoid the lengthy queue. Also book for Accademia and Duomo in Florence.
To be clear, the Borghese Gardens require no reservation. The Borghese GALLERY is visited by reservation only.
Here is the ime we are spending in Italy:
May 11- Arrive in Florence. We will have 6 full days here. Form here we will deaprt Florence and arrive to Naples by train. We will have 4 full days in Sorrento, not including arriaval and departure days. From Naples we will depart for Rome and have a full 4 days here to explore.

What time of year?
I can't see needing a car for that itinerary, but I would drop some of the time in Florence and stay somewhere is southern Tuscany. You could rent a car for that part, drive to the Amalfi coast (keep the car for a day there if you want to drive the coast). If staying in Sorrento, you don't really need a car.
