My husband and I stayed there two summers ago. We loved it. It is only accessible to San Pedro by boat, so you need to know that going in. However they have their own water taxi and if there are any tours going in or out of town they would let us travel with them at not charge. You can also take the public water taxi - I can't remember how much it costs, but for the times we were there, cost didn't seem to be an issue. We used it once or twice.
The resort itself has a beautiful pool, incredibly friendly staff and the food was good. We got some type of package that included breakfast and one or two tours. We knew we'd be out most days at some point so lunches would be out and we also knew we'd want to go into town and try different restaurants. Their beach was nice, but as many beaches in Ambergris are, there is a lot of sea grass in the water, so swimming right off the beach is a bit difficult. They have a nice pier you can swim or snorkel right from. They have kayaks you can take out and it's s short ride to Mexico Rocks, which is a reef where you can snorkel. If I had the chance, I'd go back to Portofino in a second. Personally, I liked the remoteness there.
Here's a link to my blog where there Portofino entries are - this is actually the end of the trip, so you can go backwards to find the beginning if you're interested. There are quite a few pictures of the resort and property on there.
