Bolivia Visa for US citizen

We're planning on entering Bolivia from Chile by air and will need to apply for our visas at the airport. I think we have everything we need: application form (printed out), hotel reservations, airline reservations out, yellow fever cards (original), credit card...

What will we need to have photocopies of (to leave with them)? Will they accept electronic itineraries/hotel/airline confirmation? Do we just need to show them physical yellow fever card and credit card? I'd rather not give them a copy of my credit card or bank statement. But don't want to be denied entry either.

Finally, how can we pay? Do they accept credit cards? Cash payment for a family of 4 is a lot of dollars to be carrying around.
