I don't have a tone of experience but we visited McCauly hot spring near Jemez NM na that was great but involves a 2.5 mile uphill hike.

In Ouray Colorado we stayed at the Weisbaden which has their own hot spring which heats the place including their pool. The most fun was thier Vapor Cave...not a natural cave but awesome and VERY hot.
You can pay to use the Vapor Cave even if you don't lodge there.

The nice thing about the hot springs that Ouray is famous for is that they don't have the sulfur roten eggs smell that many hot springs have. Just pure but very hot water in Ouray.
Pagosa, with it's setting on the river, is awesome.
