If I had to do this trip I'd take the bus from Naples airport to the railway station and catch the train to Taranto and hire a car there. Basically because the train is pretty spectacular, not only do you do half the route around Vesuvius but you also catch a trip up into the mountains with some fine views.
Another option is to fly into Brindisi or Bari.

Yestravel thanks for your additional advice and moral support. I was just reading an old post of mine about a northern Italy trip and it seems that our paths have crossed before in '09! Did you not love Mantova and the Palazzo Arrivebene!
